Assuming you both played exd5 on the fifth move, the board should actually look like this:
BLACK [Dianke]
A B C D E F G H
8 r n b q k b n r 8
7 p p . . . . p p 7
6 . . p . . p . . 6
5 . . . P . . . B 5

A B C D E F G H
WHITE [Wayne]
-
My bad
It should be xe5
But the board is good
-
Apart from the disappearance of Black's f-pawn? :)
-
Um.
From what I gather...
1. e4 e5
2. d4 d5
3. Bb5+ c6
4. Be2 f6
5. dxe5 ...
This post (http://shanescavestory.createaforum.com/chess/dianke-vs-wayne/msg11725/#msg11725) shows that the Pawn on d4 captured the Black Pawn on e5.
Dianke then responded by moving her pawn on f6 to capture the White Pawn on e5.
5. ... fxe5
6. Bh5+
Therefore, the black of f6 pawn didn't disappear at all...
It just somehow ended up on d4 in this post (http://shanescavestory.createaforum.com/chess/dianke-vs-wayne/msg11914/#msg11914), entirely bewildered and fighting with a dinner fork.
At least, that's what I think.
If there's a contradiction, feel free to point it out. :)
...
If I start hearing gavels in the background, then either it's on TV or I've played the Ace Attorney games too much. :P
EDIT:
... When did I get 2300 posts? :o

Fixed before I even saw your post. But thanks for noticing.

Your Queen captured a pawn on g6...
Therefore, the notation should be "Qxg6+"...
D7, my King...
